here to begin the online application process. PLEASE NOTE: This application is for licensure as a clinical social worker only. If you have not received supervision towards licensure, do not complete this application. You must apply for Registration of Supervision first.
Paper applications are available by request to the Board office at socialwork@dhp.virginia.gov.
If you are re-applying for licensure to take the clinical level examination, you must submit all documentation found within the application packet for LCSW by examination, including the verification of field placement/practicum form and the coursework outline form, as you must meet the current education and supervision requirements as outlined in the regulations.
