How do I verify my license to another jurisdiction?

Verification of a Virginia license to another jurisdiction requires a written request to the Board office with a payment of $35 per verification. Check should be made payable to the “Treasurer of Virginia”. You must provide your full name, license number and the mailing address of the jurisdiction to receive verification.

Online Renewals

The Department is currently experiencing a technical issue causing delays with our online application and online renewal process for those licenses expiring June 30, 2015. We encourage you to continue to make attempts to apply or renew online as most licensees have been able to complete the process after several attempts. Because of the high volume of online activity, we suggest subsequent attempts throughout the course of the day or evening.

In the event that you are not able to complete the process, we suggest other options for renewal which are to overnight your payment or to bring your payment in person to the office. Please note that any checks should be made out to the Treasurer of Virginia.

Late fees for June renewals will not apply until July 6, 2015. We are aware that some duplicate payments have been processed and we will refund those duplicates payments.

If you are able to delay your online application or you have a July renewal, we strongly suggest waiting until after the holiday weekend. Please note: the Department is closed Friday July 3, 2015.

Employers attempting to verify license renewals are encouraged to call the DHP automated telephone verification system (IVR) during this time of technical difficulty with online renewals at (804) 270-6836. This system is the most up to date means of licensure verification. The IVR system will verify those license records on the day that they have been successfully processed, and offer a fax back verification option. We appreciate your patience as our staff members resolve these online license renewal issues one licensee at a time.

Notice of Reporting Requirements for Dispensing

Effective July 1, 2015, all dentists who dispense controlled substances in Schedule II (Percocet, Hydrocodone), Schedule III (Tylenol with Codeine), or Schedule IV (Valium) must be registered with the Prescription Monitoring Program and must begin reporting the drugs dispensed within 7 days of dispensing. If a dentist does not dispense these products he may apply for a waiver from this reporting requirement. There is a box in this year’s dental license renewal forms to report whether you currently do or do not dispense Schedule II, III or IV controlled substances. Detailed information on the action each licensed dentist is required to take to register or to apply for a waiver will be sent out around April 1, 2015. Here are links

The US Drug Enforcement Administration published a Final Rule in the Federal Register placing hydrocodone combination products into Schedule II effective October 6, 2014. Extension of renewal requirements for deployed military and spouses

Virginia law allows active duty service people or their spouses who are deployed outside the U.S. to have an extension of time for any requirement or fee pertaining to renewal until 60 days after the person’s return from deployment.  The extension cannot last beyond 5 years past the expiration date for the license.  For more information, please read attached policy (pdf) and contact the applicable board for your license.
