Over a period of several months the Department of Health Professions will be collecting Emergency Contact Information, as required by law, from a number of different categories of healthcare professionals.
You will be notified by mail if you are required to submit this information. The notice will include instructions for providing the required information. Until you receive such notice, you will not be able to login to our system.
- If you recently received instructions from the Department of Health Professions to provide Emergency Contact Information login to the Online Licensing site to get started.
- If you are a licensed MD, DO, or DPM and have not yet provided Emergency Contact Information for the Practitioner Information Project visit the Profile site to proceed.
