Qualified Mental Health Professional (QMHP) and Registered Peer Recovery Specialists (RPRS)
- Initial Application for QMHP: You should apply with an initial application if you have completed the required education and supervised experience.
- Initial Application for QMHP-Trainee: You should apply with an initial application if you are starting or are currently accruing your supervised experience towards QMHP and have completed the required education.
- Initial Application for Registered Peer Recovery Specialist: You should apply with an initial application if you have completed the education and experience in accordance with 12VAC35-250 to provide collaborative services to assist individuals in achieving sustained recovery from the effects of mental illness, addiction, or both.
Dentist
- Licensure by Exam: You may apply for licensure by examination
if you have completed a doctoral or post-doctoral dental education program
accredited by the Commission on Dental Accreditation of the American Dental
Association and have passed a clinical examination accepted by the Board. See guidance document 60-25 Policy on Dental Clinical Competency Examination Requirements for Licensure for complete details.
- Licensure by Credentials: You may apply for licensure
by credentials if you have completed a doctoral or post-doctoral dental
education program accredited by the Commission on Dental Accreditation of the
American Dental Association, have passed a regional or state clinical examination
that involved live patients, have an unrestricted license to practice in
another jurisdiction in the United States, and have a minimum of 600 hours
of practice in each year for five out of the six years preceding an application.
Dental Hygienist
- Licensure by Exam:You may apply for licensure by examination if you have completed a dental hygiene education program accredited by the Commission on Dental Accreditation of the American Dental Association and have passed a clinical examination accepted by the Board. See guidance document 60-26 Policy On Dental Hygiene Clinical Competency Examination Requirements for Licensure for complete details.
- Licensure by Endorsement:You may apply for licensure by endorsement if you hold a current active dental hygiene license in another jurisdiction in the United States that was obtained by passing a clinical competency examination accepted by the Board, have completed a dental hygiene education program accredited by the Commission on Dental Accreditation of the American Dental Association, and have had clinical, ethical and legal practice for 24 months out of the past 48 months immediately preceding application.
Medicine
- Application: A person who is a graduate of an allopathic medical school, has passed all parts of a board-approved national exam and completed a minimum of one year post-graduate training in the United States or Canada may apply for an application to practice medicine using the Application obtained by method.
- Reciprocity: A physician (MD and DO) who has been issued a full license in one of the contiguous states/jurisdictions of Maryland and/or District of Columbia, whose license is current, active in good standing and without a license restriction in any state, has no 3 (or more) malpractice paid claims of $75, 000 or more within the most recent 10-year period, has no pending disciplinary matters or investigation by any state licensing authority, and is not currently being monitored in a physician health monitoring program.
- Endorsement: A physician (MD and DO) who has been issued a full, current and unrestricted license in good standing to practice medicine in a United States jurisdiction or Canada for a minimum of five years, has been engaged in active practice of, at least, an average of 20 hours per week or 640 hours per year for five years after postgraduate training and immediately preceding the application, and holds a current board certification issued in a medical board specialty recognized by the American Board of Medical Specialties (ABMS), Bureau of Osteopathic Specialties, American Board of Foot and Ankle Surgery, Fellowship of Royal College of Physicians of Canada, Fellowship of the Royal College of Surgeons of Canada or College of Family Physicians of Canada.
Occupational Therapy
- Application: An individual is eligible to apply for a
license to practice as an Occupational Therapist if they meet the Educational
and Examination requirements outlined in 18 VAC 85-80-10 et seq.
Osteopathic Medicine
- Application: A person who is a graduate of an osteopathic medical school, has passed all parts of a board-approved national exam and completed a minimum of one year post-graduate training in the United States or Canada may apply for an application to practice medicine using the Application obtained by method.

Radiologic Technologist & Limited-Radiologic Technologist
- Application: An individual is eligible to apply for a license to practice as a Radiologic Technologist if they meet the Educational and Examination requirements outlined in 18 VAC 85-101-10 et seq.
Respiratory Therapist
- Application: An individual is eligible to apply for a
license to practice as a Respiratory Therapist if they meet the
Educational and Examination requirements outlined in 18 VAC 85-40-10
et seq.
Registered Nurse and License Practical Nurse
- Licensure by Exam: If you are a recent graduate from
a LPN or RN state approved nursing education program that prepared you to
be a nurse and have not take the NCLEX (state board exam), you will need
to complete the application for licensure by examination. If you are a RN
or LPN educated in a foreign country, you will need to apply for licensure
by examination for LPN or RN Educated in another country. Please contact
the Board at 804-367-4515 before filing this application.
- Licensure by Endorsement: If you have graduated from
an approved nursing education program and have been licensed by examination
in another state within the U.S. or are scheduled to site for the NCLEX
in another state in the U.S., you will need to apply for licensure by endorsement.

Certified Massage Therapist
- Application: You may apply for initial certification as a massage therapist by application if you have met the education and examination requirements outlined in 18VAC90-50-40 of the regulations governing the certification of massage therapist in Virginia.
- Endorsement: You may apply for certification by endorsement if you have been licensed or certified in another state in the U.S. with requirements substantially equivalent to those outlined in 18VAC90-50-40 of the regulations governing the certification of massage therapist in Virginia and are in good standing or is eligible for reinstatement, if lapsed.
- Examination: A person who has accumulated a minimum of 1,500 hours of practical experience as a pharmacy intern
and graduated from an ACPE-approved
school of pharmacy, or a graduate of a foreign college of pharmacy who has obtained the Foreign Pharmacy Graduate Equivalency Committee
(FPGEC) certification from the National Association of Boards of Pharmacy (NABP) and obtained 1,500 hours of practical experience in the United
States shall apply for a pharmacist license using the "Licensure by Examination" obtained by method. An applicant applying by score transfer through NABP that will be completing the transfer of their NAPLEX score within NABP’s required time frame, may choose the “Licensure by Examination” obtained by method. A score transfer is valid for up to one year from the date the Board receives the score from NABP.
- Endorsement: A pharmacist who holds a current and active pharmacist license in good standing in another state shall apply for a pharmacist license using the "Licensure by Endorsement" obtained by method. When choosing the state that is the basis for transfer, please indicate the state in which you hold a current active pharmacist license. Please do NOT pick Virginia as the base state of transfer. Additionally, a license transfer application and a Virginia MPJE application will need to be completed through NABP. To avoid any delays in processing your application, we suggest completing the NABP applications at the same time you are applying for pharmacist licensure.
- Application: A person who holds a current certification from the Pharmacy Technician Certification Board (PTCB) may apply for
a pharmacy technician registration using the Application obtained by method. Additionally, a person who has a certificate
of completion for a Virginia Board of Pharmacy approved training program and has passed a Board-approved examination may apply for a pharmacy technician registration using the
Application obtained by method.
- Application: An individual is eligible to apply for a license to practice as a Pharmacy Technician Trainee if they meet the Educational requirements outlined in 18VAC110-21-135.
Practitioner of the Healing Arts to Sell Controlled Substances
- Application: An individual is eligible to apply for a license to practice as a Practitioner of the Healing Arts to Sell Controlled Substances if they meet the licensure requirements outlined in 18VAC110-30-20. This online application is not for limited use licenses for Nurse Practitioners and Physician Assistants to dispense. Please visit the Board's website for the application for these license types.
- Examination: You may apply for licensure by examination if you have meet the educational requirements specified in 18VAC112-20-40 or 18VAC112-20-50 and requirements outlined in 18VAC140-20-60.
- Endorsement: You may apply for licensure by endorsement if you hold a current unrestricted license in the United States, its territories, the District of Columbia, or Canada and meet the requirements in 18VAC112-20-65
- Doctorate: You may apply for direct access certification if you hold an active unrestricted license as a physical therapist in Virginia and shall submit evidence satisfactory to the board that you meet one of the following qualifications specified in 18 VAC112-20-81:
- Completion of a doctor of physical therapy program approved by the American Physical Therapy Association or
- Completion of a transitional program in physical therapy as recognized by the board..
- Experience: You may apply for direct access certification if you hold an active unrestricted license as a physical therapist in Virginia and shall submit evidence satisfactory to the board that you have at least three years of post licensure, active practice with evidence of 15 contact hours of continuing education in medical screening or differential diagnosis, including passage of a post course examination.
- Examination:You may apply for licensure by examination as a clinical social worker if you have met the requirements outlined in 18VAC140-20-40, education requirements prescribed in 18VAC140-20-49 and experience requirements prescribed in 18VAC140-20-50.
- Endorsement: You may apply for licensure by endorsement as a clinical social worker if you hold a current active social work license in good standing in another jurisdiction of a comparable type as the licensure that you are seeking in Virginia and have met the requirements outlined in 18VAC140-20-45.
Licensed Master's Social Worker
- Examination: You may apply for licensure by examination as a master s social worker if you have met the requirements for licensure outlined in 18VAC140-20-51 and met the education requirements prescribed in 18VAC140-20-60.
- Endorsement: You may apply for licensure by endorsement as a master s social worker if you hold a current active social work license in good standing in another jurisdiction of a comparable type as the licensure that you are seeking in Virginia and have met the requirements outline in 18VAC140-20-45.
Licensed Baccalaureate Social Worker
- Examination: You may apply for licensure by examination as a baccalaureate social worker if you have met the requirements for licensure outlined in 18VAC140-20-51 and met the education requirements prescribed in 18VAC140-20-60.
- Endorsement: You may apply for licensure by endorsement as a baccalaureate social worker if you hold a current active social work license in good standing in another jurisdiction of a comparable type as the licensure that you are seeking in Virginia and have met the requirements outlined in 18VAC140-20-45.
Registration of Supervision
An individual is eligible to apply for registration of supervision if they meet the education and experience requirements outlined in 18VAC140-20-49 and 18VAC140-20-50.
- Application: You may apply for Initial Registration of Supervision if you are applying for supervision approval for the first time, or applying for new approval after the four years of supervision previously approved has expired.
- Add/Change: You may apply for Registration of Supervision by Add/Change if you are requesting approval to addition or change a supervisor.
- Application (Examination): An applicant who has received a degree in veterinary medicine from a college or school of veterinary medicine accredited by the AVMA or have fulfilled the requirements of the Education Commission of Foreign Veterinary Graduates (ECFVG) of the AVMA or PAVE and passed the NAVLE
- Examination: Endorsement: An applicant who holds a license in another state, the District of Columbia or possessions or territories of the United States and engaged in clinical practice for the last two out of the past four years and passed the NAVLE Examination or National Board Examination/Clinical Competency Test.
- Application (Examination): An applicant who has received a degree in veterinary technology from a college or school accredited by the AVMA or the CVMA and has passed a board approved national examination for veterinary technology with a score acceptable to the board.
- Endorsement: An applicant who holds a license, certification or registration to practice as a veterinary technician in another state, the District of Columbia or possessions or territories of the United States, has been regularly engaged in clinical practice for at least two of the past four years and received a degree in veterinary technology from a college or school accredited by the AVMA or CVMA or passed a board-approved national board examination for veterinary technology.
