Missing PIN Number Request
If you've lost the Temporary PIN which was mailed to you or are requesting a PIN for the first time please complete all of the sections below and hit Submit Request. We will send you a new Temporary PIN by U.S. Mail to your address of record. If you believe that your address of record is incorrect, please contact the Board office.
