Emergency Contact Information
The Department of Health Professions collects Emergency Contact Information, as required by law, from a number of different categories of healthcare professionals.
New licensees will be notified by mail if you are required to submit this information. The notice will include instructions for providing the required information. Until you receive such notice, you will not be able to login to our system.
- If you recently received instructions from DHP to provide Emergency Contact Information or would like to update your information login to the Online Licensing site.
- If you are a licensed MD, DO, or DPM visit the Profile site to update or provide Emergency Contact Information.
