Update Your Information
Address Change Information
If you hold a license, certification or registration with the Board, please log in to our Online Licensing system to update your address. If you are an applicant or resident, please submit the Name/Address Change Form.
Name Change Information
To change your name with the Board, you must submit the Name/Address Change Form. Follow the instructions on the form and submit the required information.
A copy of one of the following documents* must accompany a name change request:
- Marriage License
- Divorce Decree
- Court Order
*Driver’s license or Social Security Number cards are not accepted
