What you'll need in order to apply:

  • Review the full set of Laws and Regulations regarding the practice of Osteopathic Medicine in the state of Virginia
  • Carefully read through the application instructions
  • Request copies of your NBOME or COMLEX transcripts at https://www.nbome.org/resources/request-official-transcript/ or by calling (866) 479-6828. Scores MAY NOT be faxed and MUST come directly from the National Board (if using the FCVS credentialing service, results will be included).
  • Contact your medical school and have them send official transcripts (with the school seal) directly to the Board.
  • Employment Activity: List all activities from the date of graduation from your professional school including but not limited to internships, employment, affiliations, periods of non-activity or unemployment, observerships, and volunteer service in the Employment Activity section of the application beginning with your first activity following professional school graduation. If you are employed by a group practice or locum tenens/traveler company, please list all locations where you have provided service or held privileges.
  • All post graduate training, including internships, transitional year, residency and clinical fellowships received in the United States or Canada must be accounted for regardless of when it was obtained or completed.

What you'll need in order to apply:

  • Carefully read through the application instructions
  • Contact the Medical Council of Canada (MCC) at (613) 521-6012 to request copies of your scores. Scores MAY NOT be faxed and MUST come directly from the MCC (if using the FCVS credentialing service, scores will be included).
  • Contact your medical school and have them send official transcripts (with the school seal) directly to the Board.
  • Employment Activity: List all activities from the date of graduation from your professional school including but not limited to internships, employment, affiliations, periods of non-activity or unemployment, observerships, and volunteer service in the Employment Activity section of the application beginning with your first activity following professional school graduation. What you'll need in order to apply:

  • Contact the  state  agency  or  licensure  board  to  have  your  scores  submitted  to  the  Scores MAY NOT be faxed and MUST come directly from the agency maintaining your score (if using the FCVS credentialing service, results will be included).

You have indicated that you meet the criteria to apply for licensure by Endorsement

What you'll need in order to apply:

  • Review the full set of regarding the practice of Osteopathic Medicine in the state of Virginia
  • Carefully read through the Endorsement Application Instructions for complete details
  • Employment - on the online application, provide a chronology of your work for the 5 continuous years prior to application with the estimated time spent practicing Osteopathic Medicine.
  • Verify that at least one license held for 5 years or more issued by another United States jurisdiction or in Canada is in good standing, defined as current and unrestricted, or if lapsed, eligible for renewal or reinstatement.
  • Board Certification - request that your certification from the appropriate entity be sent directly to the Board.
  • NPDB Self-Query - request a report from the National Practitioner Data Bank.

What you'll need in order to apply:

  • Review the full set of Laws and Regulations regarding the practice of Osteopathic Medicine in the state of Virginia
  • Carefully read through the application instructions
  • To request your ECFMG certification, follow this link https://cvsonline2.ecfmg.org/ to have your ECFMG certification provided to the Board (if you are using FCVS credentialing service, this document will be included).
  • NPDB Self Query – Complete the online “Place a Self-Query” Order form.
  • Verify your identity. This can be done electronically as part of your ”Place a Self-Query” order or by completing a paper form and having it notarized.
  • Request that official medical school transcripts be sent from your medical school to the Virginia Board of Medicine
  • If you have disciplinary action with another Board, provide a copy of the Board Order or other documentation as a separate attachment by email to medbd@dhp.virginia.gov.
  • If you have medical malpractice claims, attach a narrative that includes dates, your treatment of the patient and any payment made per settlement or judgement. You may also provide a letter from your attorney.
  • If you have misdemeanor or felony convictions provide a copy of the court documents.

Training License

Reactivate a Current Inactive Status License  

 

  • The Inactive to Active application is for licenses that are currently in a Current Inactive status. If you want to return to a Current Active status, contact the Board at 804-367-4600, Option 2 for an application.
  • If your license has been in an expired status for less than two-years, you are able to renew it using your established login credentials here. If you need assistance with your login credentials, contact our HelpDesk at 804-367-4444.
  • If your license has been in an expired status for more than two-years, will need to apply using the reinstatement application for your profession.

What you'll need in order to apply:

  • Licenses must be in expired status for more than two years
  • If you answer “yes” to any of the licensure questions from 5 through 16, provide a written explanation on a separate piece of paper and attach it to the application.
  • If you have medical mal practice claims, attach a narrative that includes dates, your treatment of the patient and any payment made per settlement or judgement. You may also provide a letter from your attorney.
  • If you have misdemeanor or felony convictions attach a copy of the court documents
  • Verification of all medical licenses from each jurisdiction within the United States, its territories and possessions or Canada in which you have been issued a full license must be received by the Board. Please contact the applicable jurisdiction where you have been issued a license to practice medicine to inquire about having documentation forwarded to the Virginia Board of Medicine.
  • NPDB Self Query – Complete the online “Place a Self-Query” Order form.
  • Verify your identity. This can be done electronically as part of your ”Place a Self-Query” order or by completing a paper form and having it notarized.
  • Provide documentation of having completed continuing education hours equal to the requirement for the number of years in which the license has been lapsed.
  • Copies of documentation supporting any name change since your initial licensure in Virginia.
  • Fill in the paper application and send to the Board, along with applicable fees.
  • If a practitioner has not engaged in active practice in his profession for more than four years and wishes to reinstate or reactivate his license, the board may require the practitioner to pass The Comprehensive Osteopathic Medical Variable Purpose Examination—USA (COMVEX-USA) given by the National Board of Osteopathic Examiners.

Licensure by Reciprocity  

You have indicated that you meet the criteria to apply for licensure by reciprocity. 

Virginia has entered into a reciprocal licensure agreement with the contiguous state/jurisdiction of Maryland and the District of Columbia. The licensure by reciprocity process allows qualified physician applicants who are currently licensed in any of these contiguous jurisdictions to forego primary source verification of core credentials and allows Virginia Board of Medicine staff to recognize a medical license issued by another party state to expedite the processing of a reciprocal license. Board staff will obtain electronic verification of licensure directly from the appropriate contiguous state or jurisdiction prior to issuing a license.

  • Review the full set of Laws and Regulations regarding the practice of Medicine in the state of Virginia.
  • Carefully read through the Reciprocity Application Instructions for complete details.
  • Verify that you hold a current, unrestricted license in the adjoining state/jurisdiction of Maryland or the District of Columbia. Board staff will obtain verification of your license directly from the applicable adjoining state/jurisdiction.
  • On the online application, provide your current practice address and the address of your projected practice location in Virginia.
  • Verify that a license issued by another United States jurisdiction or in Canada is in good standing, defined as current and unrestricted, or if lapsed, eligible for renewal or reinstatement.
