Note: The Virginia Board of Medicine accepts the verified documentation provided by the Federation Credentials Verification Service (FCVS), in case you choose to engage FCVS to help you with your application.

What you'll need in order to apply:

  • Review the full set of Laws and Regulations regarding the practice of a Physician Assistant in the state of Virginia
  • Carefully read through the application instructions
  • License Verification: Please contact the applicable jurisdiction where you have been issued a license to practice  to inquire about having documentation forwarded to the Virginia Board of Medicine.
  • Provide documentation of successful completion of a Physician Assistant program. Transcripts must be received directly from your professional school. If using FCVS, this documentation will be provided.
  • You will need a statement of current certification or letter of eligibility status directly from the NCCPA, Inc.
  • NPDB Self-Query: Complete the online Place a Self-Query Order form and verify your identity

  • Review the full set of Laws and Regulations regarding the practice of a Physician Assistant in the state of Virginia
  • Carefully read through the application instructions
  • You will need a Form C (License Verification) filled out by those jurisdictions in which you have been licensed, certified or registered.
  • NPDB Self Query – Complete the online “Place a Self-Query” Order form.
  • Verify your identity. This can be done electronically as part of your ”Place a Self-Query” order or by completing a paper form and having it notarized.
  • A completed Form B or a letter of recommendation must be received from all locations of service, places of practice or professional employment, observerships, professional research positions or professional volunteer service from the last two years.
  • The following forms are not required for approval, but it is strongly advised that you fill them out and maintain them on file, as the board may request them for additional documentation:
    • Practice Agreement for Physician Assistant and Instructions
    • Alternate Supervisors Form
    • Addendum to Practice of Physician Assistant Duties
    • Physician Assistant Invasive Procedures Protocol
 

What you'll need in order to apply:

  • You will need the company or supervising physician you are affiliated with to provide a complete list of all locations and dates where you provided service.
  • NPDB Self Query – Complete the online “Place a Self-Query” Order form.
  • Verify your identity. This can be done electronically as part of your ”Place a Self-Query” order or by completing a paper form and having it notarized.
  • A completed Form B or a letter of recommendation must be received from all locations of service, places of practice or professional employment, observerships, professional research positions or professional volunteer service from the last two years.

What you'll need in order to apply:

  • Review the full set of Laws and Regulations regarding the practice of a Physician Assistant in the state of Virginia
  • Carefully read through the application instructions
  • A notorized document from your education program certifying that you have completed 40 hours of structured didactic education instruction and at least 40 hours of supervised clinical experience that includes demonstaration of: patient dose reduction, occupational dose reduction, image recording and quality control of fluoroscopy equipment.
  • You, and your supervising physician, will need to fill out the ARRT Fluoroscopy Examination Application Form
  • Upon approval by the Board of Education, you will need to take the ARRT Fluoroscopy Examination
  • After passing the ARRT Fluoroscopy Exam, you and your supervising physician will need to fill out the Physician Assistant Authorization to use Fluoroscopy and send it to the Board of Medicine.
