Licensed Surgical Assistant - Application Wizard

Initial Application to Practice as a Licensed Surgical Assistant  

What you'll need in order to apply:

  • Completed application and associated fee.
  • Provide to the Board evidence or documented proof of one of the 3 following credentials below. You may send a notarized copy to the Board. If you are selecting number one, the Board will not accept a document that has been copied after it has been notarized.
  1. A current credential as a surgical assistant or surgical first assistant issued by the National Board of Surgical Technology and Surgical Assisting (NBSTSA), or the National Commission for Certification of Surgical Assistants (NCCSA) or their successors; or
  2. Successful completion of a surgical assistant training program during the applicant’s service as a member of any branch of the armed forces of the United States.
  3. Practice as a surgical assistant in the Commonwealth at any time in the six months immediately prior to July 1, 2020.

  3. Verification of professional credential from the NBSTSA or NCCSA
  4. Professional License Verification
  5. NPDB Self-Query

Reactivate a Current Inactive status License  

  • The Inactive to Active application is for licenses that are currently in a Current Inactive status. If you want to return to a Current Active status, contact the Board at 804-367-4600, Option 2 for an application.
  • If your license has been in an expired status for less than two years, you are able to renew it using your established login credentials here. If you need assistance with your login credentials, contact our HelpDesk at 804-367-4444.
  • If your license has been in an expired status for more than two years, will need to apply using the reinstatement application for your profession.

Reinstatement of a Lapsed License  

Reinstatements for Surgical Assistant licenses are now accepted online. Please login to the Online Licensing site to begin the Reinstatement process.

Reinstatement occurs after the license has been expired for 2 years. Do not complete a reinstatement application if your license has been expired for less than TWO years, to reactivate a license from inactive status, or if you are trying to reinstate a license that has been suspended/revoked or surrendered in lieu of disciplinary action.

Application Instructions for Reinstatement of a Surgical Assistant License

I have previously been issued a license to practice as a surgical assistant or surgical first assistant in the United States or Canada

I want to reactivate my Current Inactive license status

I want to reinstate my lapsed or expired license

   

Is your surgical assistant or surgical first assistant license in the United States or Canada is in good standing?

Do you hold a current credential as a surgical assistant or surgical first assistant issued by the NBSTSA or NCCSA or their successor?

You may not qualify for licensure as a surgical assistant by the board. Please contact the board at lsa-medbd@dhp.virginia.gov.

Do any of the following apply to you?

  • Denial of license or privilege of taking a licensure/competency exam by any testing entity or licensing authority
  • Conviction of a violation of local, state or federal statute, regulation or ordinance, or entrance into any plea agreement relating to a felony or misdemeanor. (Exclude traffic violations, except convictions for DUI and reckless driving)
  • Denial or voluntary surrender of clinical privileges for any reason
  • Placement on a corrective action plan, placement on probation or dismissal or suspension or request to withdraw from any professional school, training program, hospital, etc.
  • Termination from employment or resignation in lieu of termination from any training program, hospital, healthcare facility, healthcare provider, provider network or malpractice insurance carrier
  • Pending or past disciplinary action against your professional license/certification/permit/registration related to your practice of medicine
  • Voluntary withdrawal from any professional society while under investigation
  • Malpractice suits brought against you in the past ten (10) years
