Application Wizard - Massage Therapist

You may qualify for Licensure by Endorsement.

What you need in order to apply:

• A criminal background check (CBC) is required. For more information, see the Endorsement Checklist Instructions below.
• Please review the Laws and Regulations governing Nursing and governing the Licensure of Massage Therapists.
• You will need an official transcript mailed directly to the BON from the school you attended.
• You will need official exam results from either the Massage & Bodywork Licensing Exam (MBLEx) or the National Certification Exam (NCETM or MBLEx) sent diretly to the BON from the exam board.
• Official verification of license/certificate/verification is required from each state/jurisdiction in which you have ever been licensed/certified/registered in as a Massage Therapist. See Verification Form.
• Please read carefully through the Endorsement Checklist Instructions.

What you'll need in order to apply:

• Please review the laws and regulations governing Nursing and the Licensure of Massage Therapists.
• You will need an official transcript mailed directly to the BON from the school you attended.
• You will need to have taken the Massage & Bodywork Licensing Exam (MBLEx) and official exam results from the MBLEx need to be sent diretly to the BON from the Federation of State Massage Therapy Boards (FSMTB).
• Additional documentation may be needed, so read carefully through the Checklist Instructions.

Reinstatement as a Massage Therapist

What you'll need in order to apply:

• A Criminal Background Check (CBC) is required. For more information, see the Application Instructions below.
• Please review the Virginia Laws and Regulations governing Nursing and governing the Licensure of Massage Therapists.
• Evidence that you have completed the competency requirements in 18VAC90-50-75 of the Virginia Regulations Governing the Licensure of Massage Therapist during the period in which the license has been lapsed.
• If your current name has not been filed with the BON, you will need to submit documentation of your name change (see Checklist Instructions for details)
• If your license was revoked or suspended, additional documentation will be needed.
