Important Address Change Reminder for Registered Nurses and Licensed Practical Nurses
Virginia is a participating state in the Nurse Licensure Compact. Change of Address may affect the practice privilege for your Virginia Registered Nurse or Practical Nurse license.
A change of address will not update the practice privilege on your nursing license. The Declaration of Primary State of Residence Form is required to change the compact designation on your nursing license. "Primary state of residence” is defined by the Compact as “the state of a person’s declared fixed permanent and principal home or domicile for legal purposes.”

All licensees/applicants wishing to change their name with the Board must submit their request using the Nursing Name Change Form. Follow the instructions on the form when the required information is submitted.
Pursuant to 18 VAC 90-19-50 (B) a copy of one of the following documents must accompany a name change request:
