At least thirty days prior to the expected opening of an establishment, an owner or licensed manager seeking an establishment license should submit a completed application to the Board office. Once all documentation has been received and reviewed, the application will be forwarded to the Enforcement Division for scheduling of an inspection. The inspection process may take 15-45 days. A license will not be issued until an inspection of the establishment has been completed and approved. 18VAC65-20-170(C) Requirements for an establishment license.
Please note that the earlier the establishment or crematory can submit the application, the sooner the inspection process can be completed prior to the opening date.
An inspection is completed by Department of Health Professions (DHP) Enforcement staff to confirm the facility meets all health, safety and operational standards. An inspector from our Enforcement Division will be assigned to your establishment and contact you regarding inspection scheduling.

If the DHP Enforcement inspector identifies deficiencies or corrections needed, the manager of record will be given the opportunity to make corrections at or after the inspection. Documentation regarding deficiency corrections should be forwarded to the Board office at funeralinspections@dhp.virginia.gov within 14 days of receipt of the electronic inspection report.
A test cremation to determine the crematory equipment is functional can be scheduled by written request to the Board office after the inspection is complete. Authorization from the Board must be received prior to conducting a test cremation. 18VAC65-20-435(F) Registration of Crematories.
A confirmation email, which will include the establishment license number, will be sent to the establishment email address after the Inspection Report, deficiency corrections (if applicable) and test cremation (if applicable) documentation is approved by the Board.
