Name/Address Change FAQs

How do I notify the Board of an address and/or name change?

All address changes must be in writing, using the Name/Address Change form. The request should include your name, license number, and/or social security number, old and new addresses, and any other contact information changes. The address change may be faxed, e-mailed, or mailed to the Board office. Address changes can also be made online by visiting the Online Licensing feature.

Name changes should also be submitted with the Name/Address change form along with official documentation indicating the reason for the name change.

How do I obtain a duplicate copy of my license?

Duplicate licenses can be requested through the Online Licensing webpage.
