My license is currently listed as “Inactive.” What do I need to do to reactivate it?

To reactivate your license from inactive to active status, complete the Application for Reactivation (Inactive to Active) to Practice Physical Therapy form and provide continuing education certificates, verification of active practice, and verification of licensure from other jurisdictions, if applicable.

There is a separate requirement for a traineeship if you are unable to document verification of active practice. See 18VAC112-20-140 of the Regulations Governing the Practice of Physical Therapy for more information.

While the Board typically has a short turnaround time for completing reactivation applications, please allow up to thirty (30) days for processing.
