The program can be used as a tool for the purpose of establishing a prescription history for a specific person to assist in determining the validity of a prescription. This information may help if there is a concern by the dispenser that a patient may be seeking prescription drugs for other than legitimate purposes.

To request information from the database, the pharmacist must attest that the dispenser is in compliance with the patient notification requirements of 18 VAC 76-20-70. A request form with the patient’s name and address and attesting to the patient’s notification may be mailed or faxed to the Prescription Monitoring Program.

Requests for information will not be accepted by phone or by email.

Results cannot be provided by phone or email, but will be faxed or mailed to the requestor.

In most cases, requests will be processed the same day received.
