Prescription Monitoring Program
Prescribers
In many states with existing prescription monitoring programs, prescribers are the primary users of the information in the database. The program should be used as a tool in determining whether an existing patient or a new patient may be obtaining the same or similar drugs from other prescribers. This information may help if there is a concern by the prescriber that a patient may be seeking prescription drugs for other than legitimate purposes.
To request information from the database, the prescriber must have the specific written consent of the patient. A request form with the patient’s name and address and attesting to the patient’s consent may be mailed or faxed to the Prescription Monitoring Program.
The Presciber request form can be found here.
