How do I request a verification of my license from another state?

The Virginia Board of Long-Term Care Administrators electronically sends a verification of licensure with available public information to the jurisdiction that you designate.

To request an electronic verification of licensure, submit the Request for Verification of Virginia Long-Term Care Administrators License form with the non-refundable processing fee (check or money order made payable to “Treasurer of Virginia”).Verification requests without the required fee will not be processed.

Allow five (5) to seven (7) business days for processing. The Board verifies your profession, license number, issue date, expiration date, the method by which you obtained your Virginia licensure, and if you have any disciplinary actions against you.

You may also verify licensure information online using the License Lookup, which provides current, primary source verification of licensure.
