Please review the following before filing a complaint:
DHP does not have the authority to require a licensee to return money or provide other personal remedies.
Once a complaint is submitted to DHP, it cannot be withdrawn.
DHP is unable to provide you a copy of your complaint form once you have submitted it.
Notice Regarding Anonymous Complaints: The Department of Health Professions cannot guarantee your anonymity. If you intend to submit a complaint anonymously, do not include any information on the complaint form or supplemental documentation that reveals your identity. If you reveal your identity in any documentation you submit to DHP, you are not anonymous. Please note, however, the Department's ability to thoroughly investigate an anonymous complaint may be limited without details such as a patient name, dates of incidents, and other specific information.
