Application for Certified Rehabilitation Provider
This application is for individuals who have completed the eligibility requirements of 18VAC115-40-22, have completed the supervised experience requirements of 18VAC115-40-26 , have passed the examination requirements of 18VAC115-40-25 and would like to become a Certified Rehabilitation Provider (CRP).
Requirements for Application:
- Education:
- Completed a baccalaureate degree from a regionally accredited college or university
- Supervised Experience:
- Completed 2,000 hours of experience in performing services offered to a worker’s compensation claimant.
- Examination: Passed one of the following examinations:
- CRC Certification Exam given by the Commission on Rehabilitation Counselor Certification, or
- CDMS given by the CDMS Commission, or
- ADMS given by the CDMS Commission
Follow these steps to apply for Licensure
Step 1. Read:
- Laws regarding the Practice of Counseling in the state of Virginia.
- Regulations regarding the Certification of Rehabilitation Providers in the state of Virginia.
Step 2. Gather & Request Supporting Documentation:
- Review the Application Instructions for Certified Rehabilitation Provider (CRP) by Application for the documents needed to submit with your online application.
- Gather and request the necessary documents.
- The following are possible documents you may be required to mail with your application:
- Application for Certified Rehabilitation Provider (CRP)
- Application Fee. A $115.00 application fee is required with your application. The fee must be in the form of a check, cashier’s check or money order made payable to the “Treasurer of Virginia”.
- National Practitioners Data Bank (NPDB) Self-Query Report
- Verification of Experience for Rehabilitation Provider Certification form
- Proof of Licensure/Certification/Registration Verification
- Proof of Licensure Verification of Out-of-State Supervisor
- Proof of Name Change Documentation
- Supporting documentation related to any criminal convictions, past actions, or possible impairments.
Step 3. Mail the Application, Fee, and Supporting Documentation:
Mail the application, non-refundable fee, and required documentation to:

Step 4. Wait for Board Approval: (up to 30 days from complete application)

- You can also visit the Online Checklist to view the status of your application.
- If your name or address changes prior to receiving your certification, notify the Board as soon as possible in writing by submitting the Name/Address Change Form.
