Certified Rehabilitation Provider (CRP)-By Endorsement
Application by Endorsement
This application is for individuals who hold an active Certified Rehabilitation Counselor (CRC), Certified Case Manager (CCM), or a Certified Disability Management Specialist (CDMS) certification.
Requirements for Application:
- Hold one of the following national certifications:
- Certified Rehabilitation Counselor (CRC)
- Certified Case Manager (CCM)
- Certified Disability Management Specialist (CDMS)
- Completed 2,000 hours of supervised experience to obtain the CRC, CCM, or CDMS certification.
Follow these steps to apply for Licensure by Endorsement
Step 1. Read:
- Laws regarding the Practice of Counseling in the state of Virginia.
- Regulations regarding the Certification of Rehabilitation Providers in the state of Virginia.
Step 2. Gather & Request Supporting Documentation:
- Review the Application Instructions for Certified Rehabilitation Provider (CRP) by Endorsement for the documents needed to submit with your online application.
- Gather and request the necessary documents.
- The following are possible documents you may be required to mail with your application:
- Application for Certified Rehabilitation Provider (CRP) by Endorsement
- Application Fee. A $115.00 application fee is required with your application. The fee must be in the form of a check, cashier’s check or money order made payable to the “Treasurer of Virginia”.
- National Practitioners Data Bank (NPDB) Self-Query Report
- Verification of your Certified Rehabilitation Counselor (CRC) certification
- Verification of your Certified Case Manager (CCM) certification
- Verification of your Certified Disability Management Specialists (CDMS) certification
- Proof of 2,000 of supervised experience to obtain your CRC, CCM, or CDMS certification
- Proof of Licensure/Certification/Registration Verification
- Proof of Name Change Documentation
- Supporting documentation related to any criminal convictions, past actions, or possible impairments.
Step 3. Mail the Application, Fee, and Supporting Documentation:
Mail the application, non-refundable fee, and required documentation to:

Step 4. Wait for Board Approval: (up to 30 days from complete application)
- You will receive automated emails from coun@dhp.virginia.gov with your application status.
- You can also visit the Online Checklist to view the status of your application.
- If your name or address changes prior to receiving your certification, notify the Board as soon as possible in writing by submitting the Name/Address Change Form.
