What you’ll need in order to apply:

Submit evidence of competency to return to active practice to include one of the following:

• Information on continued practice in another jurisdiction as a licensed behavior analyst or a licensed assistant behavior analyst or with certification as a BCBA® or BCaBA® during the period in which the license has been inactive or lapsed. (You may have the employer complete and send in a Form B).
• Twelve hours of continuing education for each year in which the license has been inactive or lapsed, not to exceed three years (You will need to provide copies of certificates of completion)
• Recertification by passage of the BCBA® or the BCaBA® certification examination from the BACB.

Please fill out the application form and send to the Board, along with a check or money order made payable to the "Treasurer of Virginia" for the amount indicated on the form.

Reinstatement occurs after the license has been expired for 2 years.  Do not complete this application if your license has been expired for less than 2 years or if you are trying to reactivate a license in inactive status.

What you'll need to apply:

• Review the Laws and Regulations governing the practice of Behavior Analysis
• Carefully read through the Reinstatement application instructions
• Completed Form B-Activity Questionnaire or a letter of recommendation(see application instructions for more information).
• NPDB Self Query – complete the online “Place a Self-Query” Order form.
• Verify your identity. This can be done electronically as part of your ”Place a Self-Query” order or by completing a paper form and having it notarized.
• Verification of licenses to practice as an Assistant Behavior Analyst from all jurisdictions within the United States, its territories and possessions or Canada.
• Evidence of competency to return to active practice.
• Copies of documentation supporting any name change since your initial licensure in Virginia.
• If you answer yes to any question on the application from number 6 through number 18, provide documentation from your attorney (or you can provide a narriative explaining your answer). You will need court documentation for any convictions.
• Completed Reinstatement application and check or money order made payable to the "Treasurer of Virginia" for the reinstatement fee amount indicated on the application.
