What you'll need in order to apply:

License Verification: Please contact the applicable jurisdiction where you have been issued a license to practice to inquire about having documentation forwarded to the Virginia Board of Medicine.
Provide documentation of successful completion of a Physician Assistant program. Transcripts must be received directly from your professional school. If using FCVS, this documentation will be provided.
You will need a statement of current certification or letter of eligibility status directly from the NCCPA, Inc.

Reactivate a Current Inactive status License

The Inactive to Active application is for licenses that are currently in a Current Inactive status. If you want to return to a Current Active status, contact the Board at 804-367-4600, Option 2 for an application.
If your license has been in an expired status for less than two-years, you are able to renew it using your established login credentials here. If you need assistance with your login credentials, contact our HelpDesk at 804-367-4444.
If your license has been in an expired status for more than two-years, will need to apply using the reinstatement application for your profession.

Licensure by Reciprocity

You have indicated that you meet the criteria to apply for licensure by reciprocity.

Virginia has entered into a reciprocal licensure agreement with the contiguous state/jurisdiction of Maryland and the District of Columbia. The licensure by reciprocity process allows qualified physician assistant applicants who are currently licensed in any of these contiguous jurisdictions to forego primary source verification of core credentials and allows Virginia Board of Medicine staff to recognize a medical license issued by another party state to expedite the processing of a reciprocal license. Board staff will obtain electronic verification of licensure directly from the appropriate contiguous state or jurisdiction prior to issuing a license.

Verify that you hold a current, unrestricted license in the adjoining state/jurisdiction of Maryland or the District of Columbia.
On the online application, provide your current practice address and the address of your projected practice location in Virginia.
Verify that a license issued by another United States jurisdiction or in Canada is in good standing, defined as current and unrestricted, or if lapsed, eligible for renewal or reinstatement.
Verify that there is no pending disciplinary matter or investigation by any State Board or licensing authority in any jurisdiction.

You will need a Form C (License Verification) filled out by those jurisdictions in which you have been licensed, certified or registered.

Verify your identity. This can be done electronically as part of your ”Place a Self-Query” order or by completing a paper form and having it notarized.
A completed Form B or a letter of recommendation must be received from all locations of service, places of practice or professional employment, observerships, professional research positions or professional volunteer service from the last two years.
The following forms are not required for approval, but it is strongly advised that you fill them out and maintain them on file, as the board may request them for additional documentation:

- Alternate Supervisors Form
- Addendum to Practice of Physician Assistant Duties
- Physician Assistant Invasive Procedures Protocol

A notarized document from your education program certifying that you have completed 40 hours of structured didactic education instruction and at least 40 hours of supervised clinical experience that includes demonstration of: patient dose reduction, occupational dose reduction, image recording and quality control of fluoroscopy equipment.
You, and your supervising physician, will need to fill out the ARRT Fluoroscopy Examination Application Form
Upon approval by the Board of Education, you will need to take the ARRT Fluoroscopy Examination
After passing the ARRT Fluoroscopy Exam, you and your supervising physician will need to fill out the Physician Assistant Authorization to use Fluoroscopy and send it to the Board of Medicine.

Reinstatement

Reinstatements for Physician Assistant licenses are now accepted online. Please login to the Online Licensing site to begin the Reinstatement process.

Reinstatement occurs after the license has been expired for 2 years. Do not complete a reinstatement application if your license has been expired for less than TWO years, to reactivate a license from inactive status, or if you are trying to reinstate a license that has been suspended/revoked or surrendered in lieu of disciplinary action.
