Note: The Virginia Board of Medicine accepts the verified documentation provided by the Federation Credentials Verification Service (FCVS), in case you choose to engage FCVS to help you with your application.

What you'll need in order to apply:

  • Review the full set of Laws and Regulations regarding the practice of Podiatric Medicine in the state of Virginia
  • Carefully read through the application instructions
  • Podiatric School Transcripts:  Official podiatric medical school transcripts must be received by the Virginia Board of Medicine.
  • Jurisdiction Clearance – License Verification: Verification of medical licenses from all jurisdictions within the United States, its territories and possessions or Canada in which you have been issued a full license must be received by the Board.
  • PMLexis Scores and Federation Disciplinary Inquiry: The Board requires verification of passage of the Federation of Podiatric Medical Boards Parts I, II, Part III PMLexis Examination and the Disciplinary Report. These documents must come directly from the Federation. Please visit www.fpmb.org and click on “Order Reports” for instructions.
  • Postgraduate Training: All post graduate training, including internships, transitional year, residency and clinical fellowships received in the United States or Canada must be accounted for regardless of when it was obtained or completed. 
  • NPDB Self-Query: Complete the online Place a Self-Query Order form and verify your identity

Reactivate a Current Inactive status License  

If your license is expired, please go back to the start of the profession using the Back button at the bottom of this page, and select the license reinstatement option.

What you’ll need in order to apply:

  • An account of professional activity within the last four years, to include approximate number of hours for each year. This does not include CME activity.
  • If not engaged in active practice for more than four years, you may be required to pass one of the examinations specified in Virginia regulations 18VAC85-20-240C.
  • Completion of the continued competency requirements specified in Virginia regulations 18VAC85-20-240C.
  • Fill in the paper application and send to the Board, along with applicable fees.

What you'll need in order to apply for Reinstatement:

  • Licenses must been in expired status for more than two years.

What you'll need in order to apply:

  • Review the full set of Laws and Regulations regarding the practice of Podiatric Medicine in the state of Virginia
  • Carefully read through the application instructions
  • Verification of medical licenses from all jurisdictions within the United States, its territories and possessions or Canada in which you have been issued a full license must be received by the Board. Please contact the applicable jurisdiction where you have been issued a license to practice medicine to inquire about having documentation forwarded to the Virginia Board of Medicine.
  • Verification of passage of the Federation of Podiatric Medical Boards Part 1, Part 2, Part III PMLexis Examination and the Disciplinary Report. These documents must come directly from the Federation.
  • Contact your school of professional education and have them send your official transcripts directly to the Board of Medicine
  • List all activities from the date of graduation from your professional school including but not limited to internships, employment, affiliations, periods of non-activity or unemployment, observerships and volunteer service in the “Employment Activity” section of the application beginning with your first activity following professional school graduation. 
  • A completed employment activity Form B must be received by the Board from all locations of service, places of practice or professional employment, observerships, professional research positions or professional volunteer service for the 2 years immediately preceding application. For applicants practicing as locum tenens, have the company you are affiliated with provide a complete list of all locations and dates where you have provided service. Form B employment verifications must be received from each location of service for the past 2 years.
  • All post graduate training, including internships, transitional year, residency and clinical fellowships received in the United States or Canada must be accounted for regardless of when it was obtained or completed
  • NPDB Self Query – Complete the online “Place a Self-Query” Order form.
  • Verify your identity. This can be done electronically as part of your ”Place a Self-Query” order or by completing a paper form and having it notarized.
