Certified Surgical Technologist - Application Wizard

What you’ll need in order to apply:

Documented proof of one of the following listed five (5) pathways is needed. For any pathway selected, you can request that this information be mailed or emailed directly to the Board at surgtech-medbd@dhp.virginia.gov by the credentialing body or program where you completed your surgical technology training or apprenticeship. The Board will not accept a document that has been copied after it has been notarized.

1. Notarized copy of transcript or certificate awarded showing successful completion of an accredited surgical technologist training program and current credential as a surgical technologist issued by the National Board of Surgical Technology and Surgical Assisting (NBSTSA) or its successor; or
2. Successful completion of a surgical technologist training program during your service as a member of any branch of the armed forces of the United States; or
3. Successful completion of a surgical technologist apprenticeship program registered with the U.S. Department of Labor; or
4. Successful completion of a hospital-based surgical technologist training program approved by the Board; or
5. Successful completion of a surgical technologist training program through an accredited institution or program by a nationally recognized accreditation organization and current credential as a surgical technologist from a board-approved entity.  The surgical technologist credential issued by the National Center for Competency Testing (NCCT) and American Allied Health (AAH) are approved by the Board.

1. Application and Fee Payment
2. Employment Chronology
3. Verification of professional credential issued by the NBSTSA, NCCT, or AAH
4. Verification of a current, active Surgical Technologist License or Certification
5. NPDB Self-Query

• The Inactive to Active application is for a certification that is currently in a Current Inactive status. If you want to return to a Current Active status, contact the Board at 804-367-4600, Option 2 for an application.
• If your Virginia certification has been in an expired status for less than two (2) years, you are able to renew it using your established login credentials here. If you need assistance with your login credentials, contact our HelpDesk at 804-367-4444.
• If your Virginia certification has been in an expired status for more than two (2) years, will need to apply using the reinstatement application for your profession.

You have indicated that you are interested in the Reinstatement of your Surgical Technologist Certification.

Reinstatement occurs after the certification has expired for 2 years or more.  Do not complete this application if your certification has expired for less than 2 years or if you are trying to reactivate a license in inactive status.
