License Verifications:
The Department of Health Professions offers two methods in which one can obtain primary source verification of a Virginia license.
1. Use the free, online License Lookup service of primary source license verification.
- License Lookup is updated in real time and available 24/7 to anyone seeking licensure or other public information on any health care practitioners licensed to practice in Virginia.
- The information provided by the License Lookup service serves as an official hard copy and proof of licensure, and meets the accreditation standards for primary source verification from the top seven-accreditation organizations for healthcare professionals.
- License Lookup provides requesters with practitioner specific information including:
o License number
o Initial license date
o License expiration date
o Licensure status
o Additional public information
2. Complete the Request for Verification of Virginia License form and mail a $25.00 check or money order made payable to the "Treasurer of Virginia" to the address provided on the form. This method can take approximately 7-10 days for processing upon receipt of the mailed request.
- The Virginia Board of Social Work will provide the Verification of Licensure electronically to the jurisdiction selected on the form. The information cannot be mailed, faxed, or emailed to the licensee.
- Verification of Licensure will be provided in the standard format of the Department of Health Professions only. Forms from other jurisdictions will not be completed.
- If multiple documents are needed, a separate form and $25.00 fee must be submitted for each document.
Examination Scores: Examination test scores are not available for distribution by the Virginia Board of Social Work. You must contact the Association of Social Work Boards ("ASWB") for primary source verification at (800) 225-6880 or visit their website at www.aswb.org to request your exam results be sent to another state.
