You have selected that you would like to apply for new Cosmetic Procedure Certification.

In order for an oral and maxillofacial surgeon to perform aesthetic or cosmetic procedures, he shall be certified by the board pursuant to § 54.1-2709.1 of the Code.  Such certification shall only entitle the licensee to perform procedures above the clavicle or within the head and neck region of the body based on the licensee’s education, training, and experience, certification.

You have selected that you would like to reinstate a lapsed Cosmetic Procedure Certification.

Reinstatement of a certification that has been lapsed for more than one year shall require completion of a reinstatement form documenting continued competency in the procedures for which the surgeon is certified and payment of a reinstatement fee.

Reinstatement requirements:

• Hold an active, unrestricted dentist license from the Board
• A completed Reinstatement of Certification to Perform Cosmetic Procedures application. (Please see the instructions included with this application for full details.)
• Reinstatement Fee: A certified check/cashier's check/money order made payable to the Treasurer of Virginia in the amount of $225
• Documentation of continued competency
• ABOMS Documentation
• Current Hospital Privileges
