Application Wizard - Cosmetic Procedure Certification

You have selected that you would like to apply for new Cosmetic Procedure Certification.

In order for an oral and maxillofacial surgeon to perform aesthetic or cosmetic procedures, he shall be certified by the board pursuant to § 54.1-2709.1 of the Code.  Such certification shall only entitle the licensee to perform procedures above the clavicle or within the head and neck region of the body based on the licensee’s education, training, and experience, certification.

Requirements include:

• Hold an active unrestricted dentist license from the Board.
• Application for Certification to Perform Cosmetic Procedures: Please be sure that all information and questions are completed on the application. (Detailed instructions are included with this form)
• Application Fee: The fee for a Certification to Perform Cosmetic Procedures is $225 and must be paid with a certified check, cashier’s check or money order, made payable to the Treasurer of Virginia. 
• Original Transcript or Certification: Graduation from an Oral and Maxillofacial Surgery residency program accredited by the Commission on Dental Accreditation (CODA).
• ABOMS Documentation: Documentation verifying current board certification by the American Board of Oral and Maxillofacial Surgery (ABOMS) or documentation verifying board eligibility as defined by ABOMS.
• Current Hospital Privileges: Documentation confirming current privileges on a hospital staff to perform oral and maxillofacial surgery.
• Certification of Completion of Training: For each procedure you are applying for certification to perform, check the requirement that applies to you and attach the appropriate documentation.

You have selected that you would like to reinstate a lapsed Cosmetic Procedure Certification.

Reinstatement of a certification that has been lapsed for more than one year shall require completion of a reinstatement form documenting continued competency in the procedures for which the surgeon is certified and payment of a reinstatement fee.

Reinstatement requirements:

• Hold an active, unrestricted dentist license from the Board
• A completed Reinstatement of Certification to Perform Cosmetic Procedures application. (Please see the instructions included with this application for full details.)
• Reinstatement Fee: A certified check/cashier's check/money order made payable to the Treasurer of Virginia in the amount of $225
• Documentation of continued competency
• ABOMS Documentation
• Current Hospital Privileges
