To apply for a Restricted Volunteer Dental License:

You must meet the requirements of 18VAC60-21-230.E
Complete an Application for Restricted Volunteer Dental License along with any other required documentation (please see instructions included with the application for complete details)
Include a check or money order for $25 made payable to the Treasurer of Virginia

To apply for Registration for Volunteer Dental Practice:

You must meet the requirements of 18VAC60-21-230.F (registration of voluntary practice as an out-of-state licensed Dentist).
Complete an Application for Registration for Volunteer Dental Practice along with any other required documentation (please read the instructions included with the application for complete details).
Include a check of money order for $10.00 made payable to the Treasurer of Virginia.

You must meet the requirements of § 54.1-2726.1. Restricted volunteer license for certain dental hygienists
Complete an Application for Restricted Volunteer Dental Hygienist License along with any other required documentation (please see instructions included with the application for complete details)
Include a check or money order for $25 made payable to the Treasurer of Virginia

To apply for Registration for Volunteer Dental Hygiene Practice:

You must meet the requirements of 18VAC60-25-170 (registration of voluntary practice as an out-of-state licensed Dental Hygienist).
Complete an Application for Registration for Volunteer Dental Hygiene Practice along with any other required documentation (please read the instructions included with the application for complete details).
Include a check or money order for $10.00 made payable to the Treasurer of Virginia.
