What You'll Need in Order to Apply:

• Please review the laws (Virginia Code § 54.1-2957.04) and regulations for either initial application (18VAC90-70-60) OR by endorsement (18VAC90-70-70) regarding Licensed Certified Midwives in the state of Virginia;
• If you are a registered nurse, please return to the Applicant Resources tab and select Advanced Practice Registered Nurse to complete the application selecting the specialty of Certified Nurse Midwife.
• You will need to request verification of professional certification as a certified midwife issued by the American Midwifery Certification Board be sent directly to the Board of Nursing office by mail or emailed to: lcm@dhp.virginia.gov. A copy of the card, certificate or email verification from the applicant will NOT be accepted;
• You will need to request that an official transcript from an approved graduate degree program in midwifery be sent directly from the education institution to the Board of Nursing office by mail or email to lcm@dhp.virginia.gov; and

• Please review the laws (Virginia Code § 54.1-2957.04) and regulations for application by endorsement (18VAC90-70-70) regarding Licensed Certified Midwives in the state of Virginia;
• If you are a registered nurse, please return to the Applicant Resources tab and select Advanced Practice Registered Nurse to complete the application selecting the specialty of Certified Nurse Midwife.
