You may qualify for Registration as a Medication Aide by Examination.

If you have completed a Board approved 68 hour Medication Aide training program that prepared you to be a Medication Aide and need to take the Medication Aide Exam through PSI, you will need to complete the application for licensure by examination.

The Application for Reinstatement of Medication Aide Registration is a PDF form that will need to be printed, filled out and sent to:

What you need in order to apply:

  • Evidence of completion of all required continuing education for the period since your last renewal (see Application Instructions for more detail).
  • Attestation that there are no grounds for denial of registration.
  • If your name has changed since receiving your most current registration, you will need to submit a copy of documentation of the name change (see the Application for more detail).
  • Be sure and read the Application Instructions carefully.
